You are currently enrolled in SelectMed daily care coverage, which covers ACA-mandated Wellness & Preventive services at 100% with In-Network providers as well as doctor visits, prescriptions, and more under the Pro and Max plan levels. Formerly administered through a General Partnership, SelectMed is now only available through a Limited Partnership. In order to continue your SelectMed coverage, you will need to join the Limited Partnership.

How Do I Join the Limited Partnership?

You have received an email from “Enrollment First E-Sign” containing the joinder agreement documents necessary to join the Limited Partnership. Simply sign the documents electronically to join the Limited Partnership. Submitting these documents is required to continue your SelectMed coverage. If you cannot find the email containing these documents, call Member Services at 866-816-6786 Monday-Friday, 8:00a-7:00p EST to have them re-sent.

  • What do I have to do to keep my SelectMed plan?
    • You must join the Limited Partnership in order to continue your SelectMed coverage.
  • What is the Limited Partnership?
    • America’s Consumers & Affiliates, LP holds the contract for this group health plan. To learn more about the Limited Partnership, click here.
  • How do I join the Limited Partnership?
    • You have received an email from “Enrollment First E-Sign” containing the required documentation. Signing and submitting these documents will join you to the Limited Partnership. If you missed the initial email, a reminder will be sent 2 days later.
    • Please note: the joinder documents expire 7 days after the inital send-date. If your documents have expired, please contact Member Services for assistance.
  • What if I didn’t receive the email containing the Limited Partnership joinder documents?
    • Call Member Services to confirm your email address on file. An agent will re-send the documents to the email address you provide.
  • When is the deadline to join the Limited Partnership?
    • Enrollees must join the Limited Partnership by December 15, 2019 in order to continue coverage in 2020.
  • Will my coverage be effective if I do not join the Limited Partnership?
    • No. SelectMed is only available to partners of the Limited Partnership. Coverage will not remain effective until you have completed the Limited Partnership joinder documents.

    • Enrollment First is the benefits administrator for the Limited Partnership. Members can contact Enrollment First Member Services for any assistance pertaining to their SelectMed plan.

SelectMed Coverage Overview

SelectMed offers affordable daily medical coverage to fit the health needs of the average consumer. Thee plan levels are available: Base, Pro, and Max. All three levels cover ACA-mandated Wellness and Preventive services at 100% with in-network providers, including but not limited to: annual physicals, immunizations, well woman exams and mammograms.

The SelectMed Base plan coverage is limited to ACA-mandated Wellness and Preventive services.

The SelectMed Pro plan offers 5 Primary, Specialist, and Urgent Care doctor visits per plan year, formulary generic prescription coverage for up to 12 prescriptions per plan year, and diagnostic labs and testing.

The SelectMed Max plan offers unlimited doctor visits, unlimited prescription coverage including formulary brand prescriptions in most states, diagnostic labs and testing, as well as outpatient CT/MRI/PET scans, Mental & Behavioral Health, Rehabilitation and Habilitation services.
